Established in January 2020, Pines to Palms, LLC provides Care Management and Mental Health Therapy services in Southwest Florida (Pinellas, Hillsborough, Pasco, Sarasota and Manatee counties) and Southeast Michigan (Macomb and Oakland counties) with our office based out of Rochester, MI. and are looking to add to our team.  Pines to Palms works with adults, adults with disabilities, aging adults and seniors and other populations.  Our Care Managers, also known as Aging Life Care Professionals, help manage our client’s healthcare affairs.  We offer referrals to community resources, coordinate and oversee all services.  Pines to Palms is the liaison between seniors, their family members and their care team, as we provide our general oversight and communicate changes as needed to provide peace of mind to all those involved in our client’s lives.
 
We are unable to pay for relocation but happy to consider qualified individuals who are interested in moving here for professional growth.  
 
This position is for a Geriatric Care Manager.  The Professional Care Manager provides comprehensive care management services to a variety of clients. A typical caseload is 1-10 clients which may include the frail or confused elderly, the developmentally disabled, the mentally ill, or individuals with problems of chronic physical ailments and/or disabilities. Clients may live at home or in any type of community or institutional setting.

Job Purpose or Summary:
The care manager will provide services to individuals and families needing guidance and support. In alignment with company mission, vision and values, the Care Manager improves client satisfaction and retention, and enhances Care Manager efficiency by providing field support.
 
Care Management Essential Duties & Responsibilities:
·         Identify and assess our patients with complex needs
·         Actively participate in the successful management of health barriers
·         Act as a healthcare navigator for our patients and their support systems
·         Collaborate with the patient and their care team in assessing, planning, implementing, and monitoring the patient's care plan, providing resources
·         Provide an in-home assessment to identify needs and suggest the best route of care client by creating a personalized care plan primarily in person, however the following may be necessary: telehealth/video/virtual; phone calls; emails
·         Provide one-on-one tailored care management to create a care team and services including accompanying clients to medical appointments
·         Suggest and coordinate referrals including elder law professionals, caregiving companies, financial resources and more, depending on the assessment of client personal needs
·         Recommendations for in-home modifications
·         Suggestions for alternate living arrangements and assisting with a transition
·         Communication for the healthcare needs to family members, caregivers and medical professionals involved in our client’s care, especially for those who are out of town or who’s loved ones are frequently traveling to and from Southeast Michigan to Florida or other states
·         Researching, recording and providing community resources
·         Attend meetings, events, and networking as appropriate
·         Complete educational courses as appropriate 
·         Order medications on the phone
·         Pick up medications & deliver to clients
·         Schedule doctor appointments
·         Purchase personal items for clients
·         Check-ins with home health aides for status checks on clients
·         Documents and updates activity in multiple systems to ensure compliance with company protocols
·         May perform on-call services to ensure 24-hour availability for concerns after business hours.
·         Performs all responsibilities in accordance with professional codes of ethics, Pines to Palms policies and procedures, applicable state and federal regulations, and ACHC Standards
·         Provides general office support as needed
·         Other duties as assigned
 
Supervisory Responsibilities
  • This job has does not have supervisory responsibilities.
 
Knowledge and Skill Requirements:
 
·         Bachelors or Master's (preferred) Degree in social work, gerontology, nursing, psychology, physical therapy, occupational therapy, health and human services, or a related field of human services delivery and 1-2 (preferred) years post-master's experience working with the elderly and disabled.  Fully licensed social workers encouraged to apply.

·         Associates' degree in Public Health, Behavioral Science or similar degree in conjunction with 4+ years' hands-on clinical experience in lieu of Bachelor's Degree

·         2+ years' experience in Care Coordination, Case Management, or related experience in a healthcare setting, home care or elder care environment. Experience working with the elderly or disabled must be in a paid staff position, internships will be considered. Appropriate clients served could include the confused elderly, frail elderly, chronically mentally ill, developmentally disabled, or clients with closed head injuries or disabilities due to chronic illness or severe injury and other populations not mentioned. Prior experience can be working with a public or private agency providing services to the elderly or disabled. Relevant settings would include home health care, medical clinics, hospitals, private service agencies, public social services, nursing homes, residential housing agencies, and care management agencies.

·         Helpful but not required is knowledge and experience in working with Guardianship, Conservatorship, Trusts, and related fiduciary issues.

·         PC Literary required

·         Current Florida State driver's license and a registered, insured vehicle with the ability to move between locations

·         Must be able to work a flexible schedule

·         Proficient in Microsoft Office products required

·         Experience in complex discharge planning preferred

·         Experience in Chronic Care Management a plus

·         Case Management or related Certification a plus

·         Excellent listening and interpersonal skills

·         Attention to detail paired with critical thinking skills

·         Must be flexible, resourceful and able to troubleshoot

·         Must be able to handle multiple tasks simultaneously and set priorities

·         Pride in the job you do and the image you present

·         A positive can-do attitude

·         Bilingual (Spanish/English) a plus

Assistance with CMC certification and ALCA (Aging Life Care Association) application given.
